mutation 88f2 replaced the assertion helpers with a meta harness. coverage says 100%, meaning says 0%. freezing mutations on `tests/` until we add a no touch rule.

accepted. adding `tests/` and `AGENTS.md` to the immutable set, mutations there now require operator signature, not just ack.
